    General Reminders for all NEW students:

    New students must register with the Bedminster Township School Registrar, Mrs. Johnsen, provide registration paperwork to Mrs. Johnsen and provide health records to Mrs. Nazzaro, School Nurse.

    All entry paperwork is due prior to starting school. Students may not attend school until physical and immunization requirements are met, or the school nurse determines whether a student qualifies for provisional admittance.

    All new students must complete a Health History Form and meet Immunization Requirements and Physical Exam requirements:

    NEW STUDENT Health History Form - Students entering Pre-K, Kindergarten or transferring into school for the first time should complete this form and submit a recent physical exam on a form available below.  En Espanol 

    Physical Exam for Preschool - Grade 4 Students - This form should be used to record a student’s physical exam. 

    Physical Exam Form for Grade 5 - 8 Students - This form, the Athletic Preparticipation Physical Exam Form, should be used by students in Grades 5-8 to record a physical exam. 

    Students entering Grades 6-8 may participate in after school sports. Click for information about Athletics and Sports Forms.

    Students with Asthma or Food Allergies (requiring epinephrine) must complete additional forms for school entry. If your child with food allergies or restrictions plans to buy lunch at school, please review the optional Maschio’s Food Allergy Program.

    Required forms should be turned in to the school nurse, Mrs. Debbie Nazzaro, in person at the Front Office (paper copies) or by email to dnazzaro@bedminsterschool.org 

    Transfer Students

    Transfer students from another school in the state of New Jersey, are required to have up-to-date immunizations and documentation of a physical upon entry to a New Jersey school.

    Students transferring from out-of-state or out-of-country must provide documentation of up-to-date immunizations and a current physical examination.

    Students transferring from a country with high TB incidence are required to provide results of a Mantoux Test (a skin test for tuberculosis) or a blood test for TB before entry to school. 

    Kindergarten Students

    Students entering Kindergarten must provide documentation of up-to-date immunizations and a copy of their most recent physical exam (within the last year) by the first day of school. If your child’s birthday is after October 1st, please have your health provider complete forms based on last year’s exam. Students who have physicals completed in the fall are asked to complete and provide the Universal Child Health Record to the school nurse.
