• Maschio’s Food Allergy Paperwork

Maschio's encourages parents of students with food allergies, Celiac disease, diabetes, lactose-intolerance, or other special dietary needs to review Maschio’s Food Allergy Plan and explore their website for additional information about ingredients and menu options. Per the USDA, a food allergy may be considered a disability and require meal modification if it impacts a major bodily function or other major life activity.

For parents interested in a meal modification due to a medical diagnosis (e.g. allergies, Celiac, diabetes, lactose intolerance, etc.), Maschio's requires documentation from the child’s healthcare provider. Parents should review Maschio's Food Allergy Management Program (FAMP) and complete a medical statement verifying the student's medical diagnosis if they decide to proceed with this program Similar documentation verifying the medical diagnosis will also be accepted.

Documentation may be submitted in one of the following ways:

Click on the FOOD ALLERGY MANAGEMENT PLAN (FAMP) link below for complete information about Maschio's program and how to enroll your child.
